Peace of Mind for Aging Adults
They call it The Golden Years, a time in our lives filled with relaxation, leisure, and security. But the transition to this new chapter can be difficult and full of complex decisions for families and seniors.
Welcome to Maintain Me, a professional and personal approach to navigating the aging process. We provide services, counsel, and confidence to aging adults in the Denver Metro area seeking expert resources and peace of mind. Our goal at Maintain Me is to eliminate the stress of research and the fear of making the wrong decision. Most importantly, we prioritize a personal touch to ensure that families and seniors are being cared for as if they were our own family members.
Maintain Me offers two types of services:
1. Community Case Management: We assist those who wish to remain independent at home.
2. Transitional/Placement Services: We assist those looking to transition to independent living, assisted living or memory care.
We all have different goals for our lives in those Golden Years. Whether it is staying at home to age in place or transitioning to a higher level of care, we are always here to help! Maintain Me, Senior Services
The owner of Maintain Me is a registered nurse who takes pride in educating patients and families in the time of need. Chelsea has 8 years of experience in the health care industry including nursing and case management experience in long-term care, acute hospital settings and home health care. She offers a level of knowledge and commitment needed to best serve patients and families with todays health system. Chelsea’s passion for supporting all patients is unwavering, but her love and respect for seniors is where her true happiness lies. When deciding to start Maintain Me, her goal was to consistently assist seniors in maintaining what they define as quality of life. Whether it be modesty, independence and respect when transitioning, or providing a sense of control in a time when that may be hard to find. Chelsea wants transitioning to be an exciting new chapter in these seniors’ lives.
Morgan Jenkins comes to Maintain Me with 6 years of experience in the health care field. She spent 3 years in home health care specializing in senior care in the community. Her home health care experience provided her with a strong understanding of the resources available to those who strive to remain at home. Morgan then spent 3 years in the acute healthcare setting, learning a great deal about the challenges of our current healthcare system. During this time Morgan acted as a liaison between hospital and the patients, physicians, nurses, case managers, and administrators of the Denver Metro surrounding hospitals.
Maintain Me is proud to have Morgan as our Transition Director and we look forward to serving more seniors in the Denver Metro area!
Meet Our Social Worker
Julie began her career in case management working for children and adults with developmental delays and/or cognitive concerns, working with Federal, State and local programs. Through the changes in her personal life, she began advocating for family members as a caregiver through their aging process, including cancer, heart, dementia and Alzheimer’s. The last several years Julie has been navigating various systems, using her background as a liaison, to support a friend who had a life changing stroke through various medical, health and insurance systems. She then took her experience and provided individualized resources for people through the National Stroke Association. Julie comes to Maintain Me with her acceptance and understanding of individuals’ circumstances, providing questions to consider and knowledge they can use to make informed choices.
